SAPTX is the practice of Clayton Sponhaltz, LCDC, NCAC I. This page explains what happens to the information you share through saptx.com.

What the call-request form collects

Your name, your mobile number, where you are in the process (for example a DOT return-to-duty step, or a DWI or court- or employer-ordered evaluation), your DOT agency or that it is not a DOT matter, and anything you choose to add in the notes box.

Where it goes

When you send the form, it is emailed directly to Clayton so he can call you back. It is not stored in a database on this website, and it is never sold or shared with anyone else, including your employer. Clayton uses it only to contact you about the evaluation or SAP process you asked about.

Records consent form

The “Consent for DOT records” form linked from this website is hosted by Google Forms. It is not part of saptx.com, and what Google collects is covered by Google's privacy policy.

Cookies and tracking

This website does not use tracking cookies or analytics of its own.
